Why MD Acumen exists — and where we are going

The Problem

Medical education has a quality gap — and it is widening

The UK Medical Licensing Assessment represents the most significant reform to medical licensing in a generation. From September 2026, every doctor entering UK practice — whether a UK medical school graduate or an international medical graduate — is assessed against a single, unified standard: the GMC MLA Content Map. The bar has risen. The content has broadened from 311 to approximately 430 core conditions. The questions demand clinical reasoning, not factual recall.

Yet the preparation landscape has not kept pace. Most PLAB preparation providers offer question banks without teaching, or teaching without clinical authority, or technology without pedagogic foundation. The result is a market saturated with platforms that are adequate for revision but inadequate for genuine clinical reasoning development. The PLAB 1 pass rate in 2025 was 62%. More than one in three candidates fail — not because they lack knowledge, but because they lack the structured reasoning skills the examination is designed to assess.

For GP training, the challenge is different but equally pressing. The MRCGP AKT was reformed in October 2025 — 160 questions in 160 minutes — and the RCA demands a depth of communication and clinical reasoning that no question bank alone can develop. For practising GPs, the pace of guideline change in cardiovascular risk management, menopause, metabolic disease, and mental health demands continuous, expert-curated clinical education that goes beyond the standard CPD offering.

The gap MD Acumen was created to fill: There is no UK provider that combines genuine academic authority, AI-powered adaptive learning, expert-led live teaching, and a pedagogic framework grounded in evidence-based educational science — all delivered by clinicians who sit the examinations, treat the patients, train the GPs, and publish the research that informs them. Our Mission

To deliver excellence in postgraduate medical education

MD Acumen exists to provide the highest standard of postgraduate medical education available in the United Kingdom — for international medical graduates preparing for PLAB and the MLA, for GP trainees preparing for the MRCGP, and for practising clinicians seeking intellectually rigorous continuing professional development.

We believe that examination preparation and clinical education are not separate activities. A doctor who has been taught to reason — not merely to recall — will pass the examination and practise better medicine. Our mission is to produce doctors who do both.

Every course, every question, every clinical explanation is designed and verified by practising UK clinicians — led by Professor Rajesh Varma, a dual-CCT consultant and GP, former PLAB examiner, NICE Guideline Committee member, Cambridge visiting researcher, and active publishing academic. We are not an algorithm with a medical veneer. We are clinicians with a platform.

AI-Powered, Human-Verified

We use advanced AI to generate clinical content from current UK guidelines — NICE, CKS, BNF, and SIGN — at a scale no human-only team could achieve. But every question, every explanation, every clinical pathway is then subjected to mandatory verification by registered UK clinicians. Technology provides the scale. Reasoning First, Answers Second

Most question banks tell you which answer is correct. MD Acumen teaches you how to arrive at the correct answer — the structured clinical reasoning technique that experienced UK clinicians use instinctively and that the MLA is specifically designed to assess. The future we are building — phase by phase

MD Acumen is not a static course provider. It is a platform being built to serve the entire career arc of a UK-practising doctor — from the first PLAB examination to continuing professional development as a consultant or GP principal. The vision is phased, deliberate, and designed to scale without compromising quality.

Phase 1 — 2026

PLAB MLA Mastery Programme

The AKT Virtual Tutorial Programme launches 1 July 2026. The CPSA Clinical Skills Programme follows in November 2026. The MD Acumen adaptive question bank app launches 21 July 2026. Live courses, expert teaching, and intelligent technology — integrated from day one.

Phase 2 — 2027

MRCGP Preparation & GP Education

MRCGP AKT and RCA/CSA preparation courses designed by a practising GMC-Approved GP Trainer. The same pedagogic framework, the same clinical reasoning approach, extended into GP training and primary care education. The question bank expands to cover both PLAB and MRCGP curricula.

Phase 3 — 2027–28

Primary Care Masterclasses & CPD

Professorial-tier clinical education for experienced GPs and hospital doctors. Hot topic updates in cardiovascular risk management, menopause, metabolic disease, and clinical reasoning. Video and animation library. Podcast series. Institutional CPD licensing for NHS Trusts and ICBs.

Phase 4 — 2028+

Global Medical Education Platform

Institutional partnerships with medical schools internationally. Curriculum integration with university programmes. AI-generated personalised study plans. A global network of clinical educators delivering excellence in postgraduate medical education — anchored in UK clinical standards and academic rigour.

PLAB / MLA (AKT + CPSA)

The gateway to UK medical practice for every international medical graduate. From September 2026, fully converged with the UKMLA content map — 430 conditions, six domains, clinical vignette-based reasoning. MD Acumen will become the elite UK provider of PLAB/MLA preparation.

MRCGP (AKT + RCA/CSA)

The licensing examination for UK general practice. AKT reformed October 2025: 160 questions, 160 minutes. Written by a current RCGP-approved GP Trainer who sits in the consulting room every week. MD Acumen brings the same rigour and the same reasoning framework to GP training.
