Foundation Programme access for IMGs has fundamentally changed

The Medical Training (Prioritisation) Act 2026, which received Royal Assent on 5 March 2026, requires that UK and Irish graduates, EEA graduates, Foundation Programme completers, and those with indefinite leave to remain or EU settled status are allocated Foundation Programme places before all other eligible applicants. Non-prioritised IMGs will receive places only if posts remain unfilled after all prioritised applicants have been accommodated.

In practical terms, Foundation Programme entry via standard UKFP allocation is now severely restricted for most newly qualified IMGs. This does not close the UK to IMGs — but it fundamentally changes the career entry strategy.

UKFP Application — Now Severely Competitive

IMGs may still apply via the standard UKFP process, but prioritisation means offers are made to UK/Irish graduates first. Check eligibility and application timelines via the UKFPO website. The application window typically opens in autumn for posts starting the following August.

Trust Grade & Locally Employed Doctor Posts

NHS Trusts directly employ doctors outside the national training system. Trust Grade, Clinical Fellow, and LED posts provide salary, clinical experience, and the portfolio evidence needed for future specialty training. These are not subject to the Prioritisation Act and represent the most realistic first NHS entry point for most IMGs.

Clinical & Research Fellow Posts

12–24 month posts at NHS Trusts and academic centres. Particularly valuable for building the audit, QI, research, and publication portfolio needed for competitive specialty training applications. Often advertised directly by Trusts and via NHS Jobs.

The pragmatic strategy: Begin applying for Trust Grade and Clinical Fellow posts from the moment you have GMC registration. Do not wait for Foundation Programme allocation.
